On September 18, 2016, around 4:48 p.m., Victor Nava was driving a 2000 Honda Accord northbound on State Route 41, south of Laurel Avenue near Stratford.

At the same time, Vernon Smith was driving a 2007 Ford F-350 service truck southbound on SR 41 when Nava drove his Honda to the left and crossed over the center line, into oncoming traffic.

Smith saw the Honda enter his lane, so he swerved into the northbound lane in an attempt to avoid Nava's Honda. However, at the same time, Nava suddenly swerved back into the northbound lane and collided head on with Smith's Ford.
