Court documents obtained by 23ABC show Joanna Chavez, a former office manager with Valley Institute of Prosthetics, gave herself both an hourly wage and salary wage during her time with the company.

Part of Chavez's duties was to prepare payroll reports. She was the only employee required to prepare reports and the only employee who had access to the software. 

She was a manager from October 3, 2012 to July 7, 2017. From November 2015 to July 2017, police say she paid herself an extra $64,512.34 in 45 different payments.
